Identification and risk assessment of midwife error in the labor using systematic human error reduction and prediction approach
Authors
Abstract:
Introduction: Labor is one of the most important wards of hospital, where human error is high. Midwifery errors in the maternity ward and in the delivery can be a serious threat to the health of the mother and the infant, resulting in increased treatment costs. Factors affecting human error are diversity in work, high workload, and fatigue. Therefore, this study aimed to evaluate the midwifery errors in the maternity ward using the systematic human error reduction and prediction approach (SHERPA) technique. Material and Methods: This cross-sectional study was conducted in 2019, during which different midwifery tasks were determined in four stages of admitting, pre-labor, delivery, and postpartum. Tasks and sub-tasks were identified using the hierarchical task analysis (HTA) technique and human error was evaluated using the SHERPA technique. Results: The results of the HTA technique identified 19 main tasks, 52 sub-tasks, and 114 activities. After assessing human error risk with the SHERPA technique, the performance was the most frequent type of error and the highest frequency was related to the undesirable risk level. Conclusion: The errors of the midwifery profession in the hospital’s labor are high-risk and largely critical. Factors such as high workload, time pressure, and fatigue influence the incidence of human error. In order to reduce human error in this area, strategies such as reducing staff workload, developing standard checklists and guidelines are essential to reduce human error in this ward.

Identification and assessment of Human Error in Cabin Roofed Crane Using SHERPA and SPAR-H Techniques
Introduction: Human errors play a significant role in the occurrence of industrial accidents. This study aims to investigate the human errors in cabin roof crane operators of a metal industry using SHERPA and SPAR-H techniques. Material and Method: In this research, first, all of the tasks of the tower crane operator were identified and analyzed. Then, adopting SHERPA technique, p...
full textIdentification and Assessment of Human Error during the Process of Operating Various Types of Fire Apparatus
Background and Aim: Firefighting is a difficult and dangerous job. This job requires decision-making and speed of action in critical situations. Such conditions increase the probability of human error in the firefighting activities. Setting up fire apparatus as the first step of emergency response is associated with high criticality. The purpose of this study is identification and assessment th...

Introduction: Human factor analysis has been identified as the most common cause of accidents in natural gas transportation and distribution facilities. The occurrence of accidents at these systems, especially gas reduction stations located in residential and industrial areas, has had catastrophic consequences. Therefore, this study aimed at analyzing critical tasks and human error assessment u...

Introduction Human error is defined by Reason (1990) as the behavior of a person who did not intend to do so. It is not favorable by regulation or by observers or that the behavior causes a task or system to become beyond the accepted limits. In complex systems such as air transport, rail transport, medical services and road traffic accidents, human error has been identified as an effective fa...
full textPredictive Assessment of Driver Errors Using Human Error Template (HET)
Abstract Background an aims: Worldwide statistics declare that 1.2 million people are killed as a result of a road traffic accident (RTA). Besides, 20-50 million are injured every year. Traffic accidents account for leading causes 1.2% of deaths and 23% of injuries. In Iran, 16,000 people are annually cut short in traffic accidents. A great number of people suffer from non-fatal injuries/disab...
full textDevelopment of a human error risk assessment model in high priority emergency situation using TOPSIS, FUZZY-AHP and CREAM method
Introduction: Emergencies are unforeseen and unpredictable situations. In these situations, people’s performance is affected by various factors that cause stress. People’s performance in such situations can also affect human error probability. The purpose of this study was to evaluate human error in emergency situations based on the fuzzy CREAM and Fuzzy Analytical Hierarchy Process (FAHP). Ma...
